Medicare and Medi-Cal

Community Hospital and Montage Medical Group are participating Medicare providers. If you’re covered by Medicare or Medi-Cal, you’ll be billed for any non-covered services or those that are your responsibility.

Call your Medicare plan provider and check your Medicare Handbook for specific information about your coverage, such as which medications are covered during a hospital stay.

Billing your insurance

Plan to bring your insurance card and ID the first time you visit a Montage Health location or provider for care or if your insurance information changes.

If you have coverage from more than one insurance company, we will bill the second insurance provider after the first one has paid. If you have health insurance coverage we’re not aware of, call and let us know so we can bill your insurance directly.

How much will I pay?

Depending on your insurance coverage, you may need to pay your co-payment (copay), deductible, or estimated patient portion during your visit. Contact your insurance provider or review your insurance card to learn more.

What your insurance covers

Your insurance provider will send you an explanation of medical benefits (EOMB or EOB). It will tell you the amount your insurance paid on a claim or the reason why all or part of the claim was not paid. Once your insurance has paid its portion, you’ll receive a bill for any remaining amount due.

What your insurance may not cover

Check with your insurance provider to learn what your plan covers. Examples of hospital services your plan may not cover include:

  • Childcare services
  • Guest meals
  • Long-distance phone calls
  • Take-home medications

What if I need help to cover my bill?

If your insurance doesn’t cover the full cost of your care and you need assistance, you may:

  • Apply for financial assistance
  • Set up a monthly repayment plan. As you make payments, expect us to send you information regularly about the status of your account, including the amount currently due

Assignments of health benefits

Montage Health does not accept assignments of health benefits as payment in full of its billed charges. We reserve the right to bill patients for those billed charges that are not paid by your health plan, in accordance with the law.

Do I need pre-authorization?

Contact your insurance provider to find out whether your insurance requires pre-certification or authorization for any care at Montage Health. If it’s required and you or your doctors don’t receive authorization, your insurance company may deny the claim or not provide full benefits.

Your financial responsibility

You are responsible for any costs not covered by your healthcare plan or insurance. Read our policies about your rights and responsibilities.
