Outpatient rehabilitation

Expect the highest-quality services from Community Hospital of the Monterey Peninsula’s outpatient rehabilitation team. Outpatient means you receive therapy during short visits to one of our facilities. You and your therapist will work together to develop a care plan, including activities you can do at home to maintain your progress between sessions.

First appointment and evaluation

During your first appointment, a caring therapist will evaluate your condition in a private treatment room. Your therapist may:

  • Ask questions about your symptoms and goals
  • Perform a physical exam
  • Observe how you perform small tasks or exercises

Bring a loved one to your appointment, especially if they help you do tasks at home. You’ll both learn how to care for your needs and work toward your goals.

Treatment plan tailored to you

Based on your evaluation, your therapist will recommend a treatment plan and set a time frame for therapy. You may need to come to appointments for a few weeks, months, or longer.

Talk to your doctor

Ask your doctor for a referral for rehabilitation services — physical, occupational, and speech therapy — at Montage Health. 

Schedule an appointment

After your doctor refers you to Community Hospital of the Monterey Peninsula for outpatient rehabilitation, call (831) 883-5640 and choose option 1 to schedule an appointment.

Then, take one of these steps:

  • Print the patient intake form in English [PDF] or Spanish [PDF], fill it out, and bring it to your first visit
  • Arrive 30 minutes before your first appointment to complete the forms at your therapy location

Video visits

After your first rehabilitation appointment, ask if you can see your physical, occupational, or speech therapist through video visits from the comfort and convenience of home.
