Multidisciplinary team
You’ll be the focus of a care team made up of specialists who have different areas of expertise, including diabetes-specific qualifications. Your team may include:
- Primary care doctor who coordinates the services you receive, provides referrals, and helps you navigate the healthcare system
- Endocrinologist, a doctor who diagnoses and cares for hormone-related conditions like diabetes
- Registered nurse (RN), who holds certification in diabetes education
- Nurse practitioner (NP), who holds certification in diabetes education and management
- Registered dietitian (RD), a nutrition expert certified in diabetes education and health coaching, who teaches you how food affects blood sugar levels
- Social worker who helps you access community resources and provides emotional support
