When to get education

Because we help patients like you practice good self-care, the American Diabetes Association (ADA) endorses our program. Follow the ADA’s recommendations by getting education:

  • When you’re first diagnosed. Understand the disease, learn about treatments, set goals, and receive emotional support to live a healthy life, with fewer complications
  • During an annual assessment, to check how you’re doing and review skills to maintain good health
  • If complications develop that affect your ability to manage your diabetes
  • If your treatment plan changes or after you go to a hospital for emergency or inpatient care

  • Classes available in a private, one-on-one setting or with a group on a variety of topics, including blood sugar monitoring, medication management, diabetes complications and how to prevent them, diabetic foot and wound care, and more

Ask your doctor to fax your referral to (831) 649-7221, or call Montage Health’s diabetes team at (831) 649-7220 if you have questions about the referral process or our services. We provide appointments in both English and Spanish.
