Survivorship care plan
Talk to your care team about the long-term medical care you’ll need after you finish cancer treatment. Expect ongoing communication with your doctors because we follow the Survivorship Care Plan guidelines set by the American Society of Clinical Oncology.
Your doctor may recommend more frequent screening tests and other services to monitor your health. Count on our staff to recommend follow-up care and resources that may be right for you.
Become a patient navigator
You’re uniquely able to offer guidance to patients with cancer after you’ve completed cancer treatment. Be a source of support when you volunteer for our patient navigator program. Call (831) 625-4753 to learn more.
