Palliative care outcomes

Palliative care is a holistic approach to care that improves the quality of life of patients and their families facing the problem associated with life-threatening illness, through the prevention and relief of suffering. It includes means of early identification, assessment and treatment of pain and other problems, physical, psychosocial and spiritual. CHOMP has been awarded an Advanced Certification in Palliative Care by The Joint Commission.

There are currently no standard benchmarks for this data, but as they become available, we will include.

Pain screening
Percent of patients who are screened for the presence or absence of pain and how severe it is during their first visit with a member of the palliative care team. The screening is done by palliative care so that timely, personalized treatment is provided.
