Montage Medical Group Medicare patients with hypertension, congestive heart failure, or type 2 diabetes can enroll in Montage Health's Connected Care program, an at-home health monitoring service that remotely monitors real-time health information.

A clinical care team collects, analyzes, and responds to the patient’s vitals in real time, supporting patients with medications, labs, and coaching. Montage Health Connected Care is powered by Cadence.

What is Montage Health Connected Care?

Montage Health Connected Care provides easy-to-use devices to take vital readings from home. Our team receives a patient’s readings and automatically reviews them to make sure everything is normal. For diabetes patients, we also provide glucometers with unlimited strips and lancets.

Peace of mind for you and your loved ones

Montage Health Connected Care coordinates with your doctor to help manage your treatment in between office visits. Patients receive ongoing support when they are at home.

Your provider will still manage your care in coordination with the Cadence team. 

Montage Health Connected Care is covered through Medicare. Patients may have a monthly co-pay depending on the insurance plan.
